This printable “All About Dad” questionnaire is a fun way to get kids involved in making their own Father’s Day Gift. It’s a sweet and simple way to show Dad some extra love (and laughs).

If your husband is anything like mine, he is the hardest person to buy a gift for. My husband doesn’t ask for much, he doesn’t have tons of hobbies, and what he does want (electronics) I am woefully underqualified to pick out.
But one thing I know he does love is to be reminded that he is loved and appreciated. He works SO hard to provide for our family, and my three boys absolutely adore their awesome dad.

So in addition to giving him a couple experience gifts this year, I’m going to have my boys fill out this questionnaire about their dad.

If you’re looking for a fun way to get kids involved in making their own Father’s Day Gift, print the questionnaire below!
There’s a color version and a black & white version, so you can use whichever one works best for your printer. Also, it’s free!
It’s a great way to help kids express the reasons they love their dad, and it’s also usually pretty funny to read!
My middle son made me something similar for Mother’s Day, and I loved it! Especially when he wrote, “My mom always says…look for the good!” I guess they are listening, even when it sometimes doesn’t seem like it.
Bonus tip: Trace your child’s handprint on the pack of the questionnaire page (be sure to add the date), and have them decorate the handprint. It’s so fun to see how kids’ hands grow over time!
